ABSTRACT

Abstract Objective To explore and describe a profile of patients admitted to a psychiatric emergency facility, comparing patients with and without a recent suicide attempt in terms of their clinical characteristics and aggression. Methods This was an exploratory comparative study where patients were assessed using the Brief Psychiatric Rating Scale (BPRS) and the Overt Aggression Scale (OAS). Participants with a suicide attempt in the last 24 hours (SA) were compared to participants with a prior history of suicide attempt but no recent attempt (PHSA). Results 63 individuals (SA: 26; PHSA: 37) were selected. Both groups had similar demographic and clinical characteristics. The most prevalent diagnoses were mood (57.1%) and personality (50.8%) disorders. The majority of patients in both groups had a history of aggression episodes. Physical aggression in the week prior to admission was more prevalent in the PHSA group (51.4 vs. 19.2%, p = 0.017). The PHSA group also presented higher activation scores (p = 0.025), while the SA group presented higher affect scores on BPRS dimensions (p = 0.002). Conclusion The majority of individuals with a history of suicide attempt also presented a history of aggression. Inpatients with recent suicide attempt were hospitalized mainly due to the risk of suicide, while those with no recent suicide attempt were hospitalized mainly due to the risk of hetero-aggression. These findings support the hypothesis of an aggressive profile in suicidal patients and may open up a path for future research.


Resumo Objetivo Explorar e descrever o perfil de pacientes internados em uma unidade de emergência psiquiátrica, comparando os pacientes com e sem tentativa recente de suicídio em termos de suas características clínicas e agressividade. Métodos Trata-se de um estudo exploratório comparativo, onde os pacientes foram avaliados por meio da Escala Breve de Avaliação Psiquiátrica (BPRS) e da Escala de Agressividade Declarada (OAS). Os participantes com tentativa de suicídio (TS) nas últimas 24 horas foram comparados com participantes com história prévia de tentativa de suicídio, mas sem tentativa recente (HPTS). Resultados Foram selecionados 63 indivíduos (TS: 26; HPTS: 37). Ambos os grupos tinham características demográficas e clínicas semelhantes. Os diagnósticos mais prevalentes foram transtornos de humor (57,1%) e de personalidade (50,8%). A maioria dos pacientes em ambos os grupos apresentava história de agressão. A agressão física na semana anterior à internação foi mais prevalente no grupo HPTS (51,4 vs. 19,2%, p = 0,017). O grupo HPTS também apresentou maior ativação (p = 0,025), enquanto o grupo TS apresentou maior afetividade nas dimensões da BPRS (p = 0,002). Conclusão A maioria dos indivíduos com história de tentativa de suicídio também apresentou história de agressão. Os pacientes internados com tentativa recente de suicídio foram hospitalizados principalmente devido ao risco de suicídio, enquanto aqueles sem tentativa recente de suicídio foram hospitalizados principalmente devido ao risco de heteroagressão. Esses achados apoiam a hipótese de um perfil agressivo em pacientes suicidas e podem abrir caminho para pesquisas futuras.

ABSTRACT

Resumo OBJETIVO Descrever as estratégias de Reabilitação Psicossocial conduzidas na Rede de Atenção Psicossocial da região Oeste do munícipio de São Paulo. METODOLOGIA estudo qualitativo descritivo, realizado com 123 profissionais, de setembro de 2015 a julho de 2016. Dados processados pelo software Alceste e analisados a luz da categoria analítica Reabilitação Psicossocial de Benedetto Saraceno e literatura complementar. RESULTADOS Emergiram três classes que tratam do potencial e escassez dos serviços residenciais terapêuticos enquanto espaço de retomada da vida cotidiana; importância das atividades culturais para troca de identidades e cuidado para além do âmbito da saúde e; a potencialidade dos projetos de geração de trabalho e renda para retomada do poder contratual. CONCLUSÃO As estratégias contribuem para construção da subjetividade e retomada da cidadania. Para sustentar a Reabilitação Psicossocial é necessário superar as fragilidades de recursos humanos, físicos e estruturais. Ainda assim, há potencialidade no trabalho colaborativo e de responsabilização das equipes.


Resumen OBJETIVO Describir las estrategias de Rehabilitación Psicosocial conducidas en la Red de Atención Psicosocial de la región Oeste del municipio de São Paulo. METODOLOGÍA Estudio cualitativo y descriptivo, realizado con 123 profesionales, de septiembre de 2015 a julio de 2016. Datos procesados ​​por el software Alceste y analizados a la luz de la categoría analítica Rehabilitación Psicosocial de Benedetto Saraceno y literatura complementaria. RESULTADOS Emergieron tres clases que tratan del potencial y de la escasez de los servicios residenciales terapéuticos como espacio de retomada a la vida cotidiana; la importancia de las actividades culturales para el intercambio de identidades y del cuidado más allá del ámbito de la salud; y la potencialidad de los proyectos de generación de trabajo y renta para reanudación del poder contractual. CONCLUSIÓN Las estrategias contribuyen a construir la subjetividad y a reanudar la ciudadanía. Para sostener la Rehabilitación Psicosocial, es necesario superar las debilidades de los recursos humanos, físicos y estructurales. Sin embargo, hay potencialidad en el trabajo colaborativo y de responsabilidad de los equipos.


Abstract OBJECTIVE To describe the strategies of the Psychosocial Rehabilitation conducted in the Network of Psychosocial Care of the western region of the municipality of São Paulo. METHODOLOGY Descriptive qualitative study, carried out with 123 professionals, from September 2015 to July 2016. Data processed by the software Alceste and analyzed in light of the analytical category Psychosocial Rehabilitation of Benedetto Saraceno and complementary literature. RESULTS Three classes emerged that address the potential and scarcity of therapeutic residential services as a space to resume the daily life; importance of cultural activities for the exchange of identities and care beyond the scope of health; the potential of projects to generate work and income to regain the contractual power. CONCLUSION The strategies contribute to the construction of subjectivity and the resumption of the citizenship. In order to sustain the Psychosocial Rehabilitation it is necessary to overcome the weaknesses of human, physical and structural resources. Still, there is potential in the collaborative work and accountability of the teams.

ABSTRACT

RESUMO O presente estudo teve como objetivo compreender os sentimentos dos familiares que chegam à emergência psiquiátrica com um ente em agudização dos sintomas do transtorno mental. Trata-se de uma pesquisa descritiva-exploratória com abordagem qualitativa. Participaram 20 familiares de pessoas com transtorno mental atendidos na Emergência Psiquiátrica de um Hospital da Rede Pública do Interior do Estado de São Paulo. Para coleta de dados, utilizou-se um roteiro de entrevista individual. As entrevistas foram áudio-gravadas e tratadas conforme análise de conteúdo temática de Bardin. Os dados resultaram em duas categorias: A internação no serviço de emergência psiquiátrica como alívio da sobrecarga familiar; e A agudização do transtorno mental como gerador de angústia e tristeza para a família. Desta forma, compreender os sentimentos predominantes nos familiares dos pacientes no ato da internação no Serviço de Emergência contribui para a elaboração de estratégias para a construção de uma atuação profissional humanística visando à inclusão da unidade familiar em todas as etapas do cuidado.


RESUMEN El presente estudio tuvo como objetivo comprender los sentimientos de los familiares que llegan a urgencia psiquiátrica con un ente en agudización de los síntomas del trastorno mental. Se trata de una investigación descriptiva-exploratoria con enfoque cualitativo. Participaron 20 familiares de personas con trastorno mental atendidos en la Urgencia Psiquiátrica de un Hospital de la Red Pública del Interior del Estado de São Paulo. Para la recolección de datos, fue utilizado un guión de entrevista individual. Las entrevistas fueron audio-grabadas y tratadas conforme análisis de contenido temático de Bardin. Los datos resultaron en dos categorías: La internación en el servicio de urgencia psiquiátrica como alivio de la sobrecarga familiar; y La agudización del trastorno mental como generador de angustia y tristeza para la familia. De esta forma, comprender los sentimientos predominantes en los familiares de los pacientes en el momento de la internación en el Servicio de Urgencia contribuye para la elaboración de estrategias para la construcción de una actuación profesional humanística, pretendiendo la inclusión de la unidad familiar en todas las etapas del cuidado.


ABSTRACT This study has the objective to understand the feelings of relatives that arrive at the psychiatric emergency care unit with a family member in a moment of acute symptoms of mental disorder. It is an exploratory, descriptive research with a qualitative approach. The study has 20 families of individuals suffering from a mental disorder who were assisted in the Psychiatric Emergency Care Unit of a Public Hospital of the Interior of São Paulo state. For data collection, individual interviews were used. The interviews were audio-recorded and treated according to content analysis of Bardin. Data resulted in two categories: Internment in psychiatric emergency service to relieve the family burden, and The crisis of mental disorder such as anxiety generator and sadness for the family. Thus, to understand the feelings prevailing in the patients' relatives at the moment of hospitalization in the Emergency Care Unit, there was the elaboration of strategies for the construction of a professional humanistic performance seeking the inclusion of the family unit in all stages of the care.

Objetivos Descrever as principais características de vítimas de tentativas de suicídio atendidas em instituições públicas de saúde de Fortaleza-Ceará, Brasil. Método Estudo quantitativo, utilizaram-se um questionário com perguntas estruturadas e Inventário de Depressão de Beck. Trabalhou-se com amostra por conveniência com 360 vítimas nos Centros de Atenção Psicossocial Geral, Atenção ao usuário de álcool/ drogas, Assistência à criança/adolescente, Hospital Doutor José Frota e Projeto de Apoio à Vida. Resultados Na análise estatística, usouse como variável dependente mais de uma tentativa de suicídio, associada às variáveis independentes. Na análise multivariada com mais de uma tentativa, e com significância (p<0,05), mantiveram associação: sexo masculino, OR=2,1 (IC95 %:1,2-3,6), p=0,005; sentimento de rejeição, RC=2,4 (IC95 %:1,4-3,9), p=0,001; internamento em hospital psiquiátrico, RC=3,4 (IC95 % :2,0-5,7), p=0,000; acreditar decepcionar alguém, RC=2,4 (IC95 %:1,3-4,4), p=0,005; depressão, RC=1,0 (IC95 %:1,0-1,0), p=0,001. Conclusão Os dados apontam a necessidade de maior atenção a essa população, no sentido de promover diferenciados serviços de apoio, quer psicológico, quer psiquiátrico para melhoria de vida das pessoas.


Objective This study aimed to describe the main characteristics of victims of suicide attempts treated at public health units of Fortaleza-Ceara, Brazil. Methods With a quantitative approach, we used a questionnaire with structured questions and the Beck Scale for depression. We worked with a convenience sample of 360 victims in Psychosocial Care Centers (General/Alcohol-Drug/Infant to Teen), Doutor José Frota Hospital, and the Apoio à Vida Project . Results In the statistical analysis, the dependent variable of more than one suicide attempt was used, associated to the independent variables. Multivariate analysis, with more than one attempt and significance (p<0.05), maintained association: male OR=2,1 (IC95 %: 1,2-3,6), p=0,005; feeling rejected, RC=2,4, (IC95 %:1,4-3,9), p=0,001; admission to psychiatric hospital, RC=3,4 (IC95 %:2,0-5,7), p=0,000; believing to have disappointed someone, RC=2,4 (IC95 %:1,3-4,4), p=0,005; depression, RC=1,0 (IC95 %: 1,0-1,0), p=0,001. Conclusion The data shows the need for greater attention to this population, in the sense of promoting differentiated service support, either psychological or psychiatric, to improve people's lives.


Objetivos Este estudio tuvo como objetivo describir las principales características de las víctimas de intentos de suicidio atendidas en instituciones de salud pública en Fortaleza-Ceará, Brasil. Métodos Estudio cuantitativo, se utilizó un cuestionario con preguntas estructuradas y el Inventario de Depresión de Beck. Se trabajó con una muestra de conveniencia con 360 víctimas en los Centros de Atención Psicosocial General, Atención al usuario de alcohol/drogas, la asistencia a los niños/adolescentes, Hospital Instituto José Frota y el Proyecto de Apoyo a la Vida. Resultados En el análisis estadístico, se utilizó como variable dependiente más de un intento de suicidio, asociado con las variables independientes. En el análisis multivariante, con más de un intento, y la significación (p<0,05), se mantuvo asociada: hombre, OR=2,1 (IC95 %:1,2-3,6), p=0,005; sentimiento de rechazo, OR=2,4 (IC95 % :1,4-3,9), p=0,001; ingreso en el hospital psiquiátrico, OR=3,4 (IC95 % :2,0-5,7), p = 0,000; creer decepcionar a alguien, OR=2,4 (IC95 %:1,3-4,4), p=0,005; depresión, OR=1,0 (IC95 % :1,0-1,0), p=0,001. Conclusión Los datos apuntan la necesidad de una mayor atención a esta población, en el sentido de promover diferenciados servicios de apoyo, sea psicológica, sea psiquiátrica para mejorar la vida de las personas.

ABSTRACT

CONTEXT AND OBJECTIVE: The prevalence of psychiatric conditions in clinical settings is high, particularly in emergency services. This is a challenge for healthcare professionals and an essential element in the functioning of the mental health network. The objective here was to describe the sociodemographic and clinical profile and the practices among patients treated psychiatrically in the Emergency Unit. DESIGN AND SETTING: Descriptive and quantitative study, conducted at Hospital das Clínicas (HC), Universidade Estadual de Campinas (Unicamp). METHODS: Sociodemographic data, reasons for attendance, diagnostic hypotheses and practices were analyzed. RESULTS: Psychiatric staff attended 1,835 cases over the study period, corresponding to 1465 patients. The patients were predominantly women (53.7%) and white (79.6%); their mean age was 37 years and 41.3% lived with their parents. The commonest reasons for attendance were depressive symptoms (28.1%), agitation (23.6%) and problems with psychoactive substances (19.5%). The commonest diagnoses were psychoactive substance-related disorders (23%) and depressive disorders (18.5%). 31.6% of the patients were referred to healthcare centers and 29.2% to specialized outpatient clinics, while 8.2% were hospitalized. CONCLUSIONS: This study emphasizes that it is important for professionals working in emergency service to have information about the patients' profile and the main reasons that lead them to seek psychiatric care, and to establish a diagnosis that will allow proper management at the emergency service and case referral. .


CONTEXTO E OBJETIVO: A prevalência de quadros psiquiátricos em contextos clínicos é elevada, particularmente em serviços de emergência, representando um desafio para os profissionais da saúde e um elemento essencial no funcionamento da rede de saúde mental. Objetivou-se descrever o perfil sócio-demográfico, clínico e condutas para pacientes atendidos pela psiquiatria na Unidade de Emergência. TIPO DE ESTUDO E LOCAL: Estudo descritivo e quantitativo, conduzido no Hospital das Clínicas (HC), Universidade Estadual de Campinas (Unicamp). MÉTODOS: Foram analisados dados sócio-demográficos, motivos de atendimento, hipóteses diagnósticas e condutas. RESULTADOS: Houve 1.835 atendimentos pela Psiquiatria no período, correspondendo a 1.465 pacientes. Encontrou-se predomínio de mulheres (53,7%), idade média de 37 anos, maioria branca (79,6%); 41,3% moravam com os pais. Os motivos mais frequentes de atendimento foram sintomas depressivos (28,1%), agitação (23,6%) e problemas com substâncias psicoativas (SPA) (19,5%). Os diagnósticos mais frequentes foram transtornos relacionados a SPA (23%) e transtornos depressivos (18,5%). 31,6% dos pacientes foram encaminhados para centros de saúde, 29,2% para ambulatórios especializados e 8,2% foram internados. CONCLUSÃO: O estudo reforça a importância de que profissionais que atuem em serviços de emergência tenham informações sobre o perfil, os principais motivos que levam os pacientes a procurar atendimento psiquiátrico, além do estabelecimento de uma hipótese diagnóstica que permita uma adequada conduta no serviço e o seu encaminhamento. .

OBJECTIVES: To evaluate the compliance to the prescribed drug treatment and referral of patients discharged from a psychiatric emergency service (PES). METHOD: From a total of 330 patients enrolled in the study, 175 (53%) agreed to a telephone inquiry 60 days after the PES visit regarding the status of the prescribed medication use and the outpatient referral. RESULTS 227 patients (68.8%) received prescription for a psychotropic medication and all patients were referred to an outpatient psychiatry service. Of the 175 patients who agreed to participate, 153 (87.4%) were successfully contacted by phone. Out of these, 97 patients (63.4%) were using the prescribed medication and 83 (54.2%) had scheduled a community appointment after 60 days. Patients who received a prescription had a greater chance of being on psychotropic medications at follow-up (OR 2.88; IC 95% 1.33-6.22; p = 0.007). However, the prescription was not associated with being in regular outpatient treatment (OR 0.76; IC 95% 0.036-1.61; p = 0.475). CONCLUSIONS: Psychotropic medications were routinely prescribed for PES patients, but this practice did not increase compliance to outpatient treatment referral after two months.


OBJETIVOS: Avaliar a adesão ao tratamento farmacológico prescrito e ao encaminhamento em pacientes atendidos em um Serviço de Emergências Psiquiátricas (SEP). MÉTODO: Foram coletados dados demográficos e clínicos de 330 pacientes atendidos em um SEP durante um ano. Sessenta dias após o atendimento, a adesão ao tratamento farmacológico e a continuidade do tratamento em caráter ambulatorial foram avaliadas através de contato telefônico. RESULTADOS: 227 pacientes (68,8%) receberam prescrição de medicações psicotrópicas. Todos os pacientes foram encaminhados para serviços psiquiátricos ambulatoriais. Cento e setenta e cinco pacientes concordaram em receber o contato telefônico. Desses pacientes, 97 (63,4%) estavam usando a medicação prescrita e 83 (54,2%) haviam marcado consulta após 60 dias. Os pacientes que receberam uma prescrição tiveram maior chance de estar em uso de medicações psicotrópicas no contato do seguimento (RC 2,88; IC 95% 1,33-6,22; p = 0,007). Entretanto, a prescrição não foi associada ao agendamento de consulta ambulatorial (RC 0,76; IC 95% 0,036-1,61; p = 0,475). CONCLUSÕES: Medicações psicotrópicas são prescritas rotineiramente para pacientes atendidos em SEP, mas esta prática não aumentou a adesão ao encaminhamento para tratamento ambulatorial após dois meses.

Introducción: Una urgencia psiquiátrica es cualquier alteración del afecto, conducta o pensamiento que puede producir daños a terceros o al consultante. La consulta de urgencia ido en aumento en los últimos años. Objetivos: Determinar la prevalencia de las distintas patologías, describir su distribución según sexo, edad, sector de residencia, estado civil, ocupación, previsión y derivación en las consultas de urgencia psiquiátrica en el Hospital Psiquiátrico "Dr. JoséHorwitz" de Santiago y comparar estos resultados con publicaciones previas de estudios en el mismo centro. Material y Método: Muestra representativa de 1999 casos, escogidos aleatoriamente de las consultas realizadas entre el Iº de julio de 2005 y el 30 de junio de 2006. Se agruparon los diagnósticos de acuerdo a la nomenclatura CIE-10. Resultados: 48,7 por ciento de las consultas fueron realizadas por pacientes de sexo masculino. La media de edad fue 39 años. 76,7 por ciento de las consultas fueron realizadas en horario diurno. El 48,3 por ciento de los pacientes no tiene ocupación. 48 por ciento eran solteros. Abuso de sustancias y alcohol, trastornos esquizoides, afectivos y neuróticos congregan el 73,9 por ciento de las consultas. La más prevalente fue la de trastornos asociados al abuso de sustancias y alcohol con un 19,7 por ciento del total. Conductas autoagresivas (74 por ciento) y trastornos afectivos (74 por ciento) se asoció más a mujeres, en cambio el abuso de sustancias y alcohol más con hombres (76 por ciento). Discusión: Las consultas por trastornos esquizoides han disminuido a lo largo de los años. En cambio las consultas por el abuso de sustancias y alcohol y los trastornos afectivos han aumentado. Se hacen sugerencias para enfrentar el nuevo perfil epidemiológico.


Introduction: A psychiatric emergency is any alteration of affection, thought or behavior that may cause harm to others or to the same consultant. These consultations have increased over the last years. Objectives: Determine the prevalence of the various pathologies, describing their distribution according to sex, age, area of residence, marital status, occupation, health insurance, and referral in psychiatric emergency consultations at "Dr. José Horwitz" Hospital of Santiago, and to compare these results with previously published studies in the same center. Material and Method: We used a representative sample of 1999 cases, chosen randomly from consultations held between July 1,2005 and June 30,2006. Diagnoses were grouped according to the nomenclature of the ICD-10. Results: 48.7 percent of the consultations were carried out by male patients and. Average age of consultation was 39. 76.7 percent of consultations were carried out in daytime. The 48.3 percent of people who consulted didn't have any occupation. 48 percent were singles. Abuse of substances and alcohol, schizoid, afective and neurotic disorders congregate 73.9 percent of total consultations. The most prevalent were the disorders associated with abuse of substances and alcohols with a 19.7 percent overall. Autoagressive behaviors (74 percent) and affective disorders (74 percent) were associated with females, while abuse of substances and alcohol was associated with males (76 percent). Discussion: Schizoid disorders have proportionally declined in importance over the years. In contrast, abuse of substances and alcohol, and affective disorders have increased their frecuency. Suggestions are made to FACE the new epidemiological profile.

ABSTRACT

Se realizó un estudio descriptivo desde Octubre a Diciembre de 2008 en la Emergencia del Hospital Vilardebó estableciéndose la prevalencia del consumo de sustancias psicoactivas en los pacientes que ingresaban por Trastornos Psicóticos. La presencia de consumo reciente se estableció por autorreporte y test en orina para marihuana y cocaína/ pasta base de cocaína. La prevalencia del consumo reciente en los pacientes con Trastornos Psicóticos fue de 17,9 por ciento. Los pacientes con testeado positivo mostraron una tasa total de ingresos mayor que aquella del grupo de los no consumidores, y los ingresos en el último año fueron claramente mayores para el grupo que consume cocaína y sus derivados.


A descriptive study was conducted from October to December 2008 in the emergency room of Hospital Vilardebó (State Mental Facility) to establish the prevalence of psychoactive substance use in patients admitted for Psychotic Disorders. The presence of recent use of substance was established by self-report and urine test for marijuana and cocaine / cocaine base. The prevalence of recent use in patients with psychotic disorders was 17.9 percent. Patients who tested positive showed a total hospitalization rate greater than that of the group of nonusers, and hospitalization in the last year were clearly higher for the group who uses cocaine and its derivatives.

ABSTRACT

Introducción: La evaluación de modelos de atención en salud mental es compleja, existiendo carencias de estudios de impactos de estos servicios; también del Modelo Comunitario de Atención en Salud Mental (MCASM). La Consulta de Urgencia Psiquiátrica (CUP) se utiliza como indicadorde resultado: reúne emergencia real y demanda no resuelta. El Complejo Asistencial Barros-Luco (CABL) constituye un modelo naturalístico de comparación: provee CUP a 2 servicios de salud: Sur (SSMS) y Sur-Oriente (SSMSO), el primero con mayor desarrollo del MCASM. Del mismomodo las comunas del SSMS presentan distintos grados de implementación del MCASM. Objetivo: Comparar la variación de tasas de CUP en el CABL, según SS de procedencia y comuna delSSMS. Metodología: Estudio observacional retrospectivo (años 2006-2007). Se compararoncomunas del SSMS, categorizándolas por presencia de MCASM y distancia al CABL usando modelos de regresión de poisson. Resultados: Se analizaron 11.760 CUP. Existe caída de tasasde CUP, proporcionalmente mayores en SSMS y en comunas con MCASM. Sin embargo, comunas con MCASM presentan Razón de Tasas de Incidencia (RTI) de CUP mayores al doble. Al ajustar por distancia y años, la RTI disminuye (1,38 [96%IC 1,07-1,77]). Hay diferencias en caída deCUP entre comunas, no explicables por los factores estudiados. Discusión: Mayor desarrollo del MCASM podría asociarse a disminución de CUP. Hay limitaciones: periodo de observación corto, ausencia de ajustes por otros confundentes, categorizaciones poco precisas.


Introduction: Health care model evaluation in mental health is complex, and there is a lack of impact studies on those services, as well as on the Community Mental Health Care Model (CMHC). Emergency Psychiatric Consultation (EPC) is used as an indicator of results: it includes both real emergencies and unresolved demand. The Barros Luco Health Care Complex (BLHC) provides an opportunity to study the impact of the CMHC model: it provides EPC to two health services, South (SHS) and South-East (SEHS). The former has more widely implemented the CMHC model, and the counties of the SHS range in their implementation of the CMH model. Objective: Compare the variation in rates of EPC in the BLHC by health service of origin, and by county within the SHS. Methodology: This is an observational retrospective study (2006-2007). Counties within the SHS were categorized by implementation of the CMH model and distance to the BLHC, and analyzed using Poisson regression models. Results: 11,760 EPC were analyzed. A decrease in EPC rates can be observed, which is greater in the SHS and in the counties that have implemented the CMHC model. Nevertheless, counties with the CMHC model had incidence rate ratios (IRR) for EPC that were more than twice as high. After adjusting for distance and years, the IRR decreased (1.38 [96 per cent CI 1.07-1.77]). There are differences in the decrease in EPC between counties that are not explained by the factors studied. Discussion: Greater development of the CMHC model could be associated with the decrease in EPC. There are limitations: short observation period, lack of adjustments for other confounding factors, imprecise categories.

A propósito de la solicitud de Autorización Sanitaria para el Servicio de Urgencias del Instituto Psiquiátrico “Dr. José Horwitz Barak”, se describen los pasos realizados en el proceso y algunos de los resultados, proponiendo la puesta en marcha de innovaciones necesarias para un mejor servicio a los pacientes.


Under the requirements of Sanitary Approval from the Health authorities, the Emergency Service of a Psychiatric Hospital implemented a task force to review technical as well as legal procedures used and updated them. Some suggestions to improve quality of care are recommended.

Se estudió la demanda psiquiátrica en la Asistencia Pública de Santiago durante mayo de 2005. Se registraron las variables sociodemográficas y clínicas extraídas del formulario de atención. Los diagnósticos se organizaron de acuerdo al CIÉ 10. Se encontraron 562 consultas equivalentes al 5,1 por ciento de las atenciones realizadas. Destaca que: 52 por ciento eran mujeres; 48 por ciento eran menores de 35 años 54,6 por ciento provenían del Área Central y 60 por ciento tenía gratuidad. El 58 por ciento consultó de 8 a 20 hrs. En términos diagnósticos, destacaron los trastornos por alcohol y drogas (32,7 por ciento) y los trastornos neuróticos (51,1 por ciento). En los hombres predominaron los trastornos por alcohol y drogas (58,2 por ciento) y los trastornos neuróticos (31,9 por ciento) de los cuales los trastornos somatomorfos eran el 62,8 por ciento; mientras que en las mujeres fueron los trastornos neuróticos (68.7 por ciento), los trastornos por alcohol y drogas (9,3 por ciento) y las autolesiones intencionales (10 por ciento). Se indicó psicotrópicos al 15 por ciento de los consultantes e interconsulta a especialista sólo en un 2,5 por ciento. La frecuencia de trastornos es similar a la observada en estudios previos. Se constata escasa derivación a especialista o atención psicológica especializada en el establecimiento.


We studied the psychiatric demand in Hospital Asistencia Pública Dr. Alejandro del Rio, during May of2005. We registrated social, demographic and clinical variables, taken from the attention data. The diagnostic were organized following CIE10 classification. We founded 562 psychiatric attentions, which means 5.1 percent of all the attentions done in this institution. It's remarkable that 52 percent of the patients were women, 48 percent) younger than 35 years, 54.6 percent) were from metropolitans center area and 60 percent > had free attentions. A 58 percent> consulted at daytime (08:00 - 20:00hrs). In the whole sample we founded a high number of drugs and alcoholic disorders (32.7 percent >), and also a high incidence of neurotic disorders (51.1 percent >). In the men group predominated drugs and alcoholic disorders (58.2 percent >), followed by neurotic disorders (31.9 percent); in this last group a 62.8 percent correspond to somatomorphic disorders. Inside the women group the highest incidence was for neurotic disorders (68.7 percent), followed by self- aggressive disorders in 10 percent, and in third place the drugs and alcohol disorders (9.3 percent). They gave psychotropics drugs to a 15 percent of the patients and they sent to specialist consult only to a 2.5 percent. Conclusions: The disorders frequency founded now is similar to the frequency seen in others investigations. We noticed a very low derivation to psychiatric specialists.

ABSTRACT

Blackground: Five percent of children and adolescents consults in mental health services in one year. Approximately one every ten children has a mental health problem. Aim To assess clinical and demographic factors of children and adolescents hospitalized by psychiatric cause in a university psychiatric clinic. Patients and Methods: Review of medical records of 167 subjects aged 9 to 17 years, 97 women, admitted to a Psychiatric Service in the period 2001-2004. The International Classification of Primary Care (ICPC) manual of the World Organization of National Colleges, Academies and Academic Associations of General Practitioners/Family Physicians (WONCA) was used to classify admission complaints and symptoms and DSMIV to classify psychiatric disorders. Results: Mean hospital stay was 11+8 days. The main admission causes were suicidal attempts and psychomotor agitation/impulsive behavior in 54 percent and 26 percent of cases, respectively. The main psychiatric diagnoses were depressive disorder, suicide attempt and bipolar disorder. In 69 percent of patients, the personality diagnosis was deferred. Only 25 percent of families were considered functional. Conclusions: Affective disorders and suicidal behavior are the main psychiatric diagnoses at discharge in children and adolescents admitted to a psychiatric impatient service.

ABSTRACT

En la evolución de su enfermedad algunos pacientes presentan tal grado de peligrosidad, fugas reiteradas, rechazo reiterativo del tratamiento que hacen insostenible su atención con los procedimientos habituales. Remodelado al servicio de urgencia, concebimos la Unidad de Cuidados Psiquiátricos Permanentes, Unidad virtual, cuya utilización para esos enfermos durante cinco años comunicamos en este trabajo. La población constituida por 12 hombres y 6 mujeres, en su mayoría solteros y jóvenes, ingresados principalmente por violencia y fugas reiteradas desde los servicios de agudos, sufrían especialmente de esquizofrenia y abuso de drogas. Dentro de un rango de cinco a noventa y siete días, 32 por ciento permaneció menos de 15 días, y el 27 por ciento más de un mes. En cuanto a los tratamientos el 71 por ciento permaneció aislado en la unidad , recibiendo psicofármacos el 100 por ciento, TEC el 54 por ciento y cirugía estereotáxica el 15 por ciento. Al terminar el período considerado, el 60 por ciento continuaba su tratamiento, igual cantidad permanecía con su familia, sin perfuicio de que el 90 por ciento había vuelto a hospitalizarse en un servicio clínico. Esta experiencia demuestra la posibilidad de tratar, bajo condiciones de aislamiento, a pacientes de alta complejidad y sugiere que la existencia de un lugar como la UCPP es deseable

ABSTRACT

Materials and methods: A random sample of 439 patients charts, corresponding to 3 percent of all consultations during 1994, were analyzed. The results were compared with consultations during 1998, analyzed in a previous study. Results: During 1994, patients were older, came alone to the emergency wards and consulted during the night with greater frequency than during 1988. In both years psychosis were the main cause of consultation. Compared to 1988, during 1994 there was an increase in the frequency of affective disorders (16 and 12 percent of all consultations respectively) and substance abuse (15 and 3 percent respectively). Conclusions: The increase in consultations due to affective disorders and substance abuse in 1994 is in accordance with changes in the epidemiological profile of psychiatric disorders in Santiago

Con el fin de conocer la realidad de la atención de urgencia en psiquiatría infantojuvenil, se procede a revisar dos grupos de pacientes atendidos en el Servicio de Psiquiatría del Hospital Dr. Exequiel González Cortés del área sur de Santiago, en 1983 y 1990. Se analizan los resultados encontrando grupos de riesgo y una frecuencia distinta de patologías y edades de presentación. Se discuten las posibles causas y se proponen algunas líneas de acción para programas futuros

ABSTRACT

Este trabajo comunica los resultados de una investigación sobre la estructura de la demanda al único servicio psiquiátrico de urgencia público de Santiago, dependiente del Instituto Psiquiátrico "Dr. José Horwitz Barak", el centro especializado más grande de la metrópolis. Preferentemente jóvenes y solteros, hombres y mujeres en cantidades casi iguales, acuden reiteradamente desde todas las comunas de la capital. A pesar que afirman vivir junto a familiares, la cuarta parte de los pacientes llegan solos al servicio. Los diagnósticos más frecuentes fueron la psicosis (31,6%), la neurosis (21,6%) y las alteraciones de la personalidad (10,2%), apreciándose diferencias significativas entre ambos sexos. La morbilidad influye en el tipo de tratamiento siendo lo más frecuente la utilización de neurolépticos y benzodiazepinas. La hospitalización limitada por escasez de camas, se reserva a la patología psicótica más severa, cualquiera sea el lugar de residencia del paciente. El control en el mismo servicio y la citación al consultorio del Instituto son otras medidas de derivación, siendo escaso el traslado a otras instituciones. La falta de camas en otros lugares y en general la pobreza de los recursos en salud mental, explican esta situación y justifican los esfuerzos que se hagan por mejorarla. En el marco de este trabajo, parece necesario impulsar el desarrollo de la atención psiquiátrica de urgencia en otros hospitales generales de Santiago
